Investigations into the tragedy in India have made a frightening discovery about the crash of an Air India plane that killed 260 people in June.
Just seconds after takeoff, the two fuel control switches on the Boeing 787 Dreamliner, which had been in operation for 12 years, suddenly disengaged, leaving the engines without fuel and causing a total power outage.
This procedure is usually done only after a plane has landed. The cockpit voice recording shows one pilot asking the other why he “made the cut-off,” to which he replies that he did not. The recording does not clarify who said what. At the time of takeoff, the co-pilot was at the controls of the plane, while the captain was monitoring.
The keys returned to their normal position during the flight, causing the engine to automatically shut down. At the time of the crash, one engine was regaining power, while the other had recovered but had not yet regained power. Air India Flight 171 remained airborne for less than a minute before crashing into a crowded neighborhood in the western Indian city of Ahmedabad, marking one of India's strangest aviation accidents.
The Air India plane climbed 625 meters into the air in clear weather before losing its location data about 50 seconds later, according to the website FlightRadar24. The preliminary report from the investigation — led by Indian authorities with experts from Boeing, General Electric, Air India, Indian regulators and representatives from the United States and the United Kingdom — raises several questions. Investigators say the lever-type fuel switches are designed to prevent accidental activation; they must be pulled up to unlock, a safety feature that dates back to the 1950s.
