Ozzy Osbourne, the heavy metal icon and legendary former vocalist of Black Sabbath, passed away on July 22 at the age of 76 – and now the official cause of his death has been confirmed.

According to the death certificate, published by the New York Times, Osbourne died from: Out-of-hospital cardiac arrest,
acute myocardial infarction, coronary artery disease, Parkinson's with autonomic dysfunction.

The documents were filed by his daughter, Aimee Osbourne, while his family had previously announced his passing through a brief statement.

Speculation has been rife in recent weeks about his deteriorating condition. Osbourne's other daughter, Kelly, was forced to deny rumors of a "suicide pact" between Ozzy and his wife, Sharon, saying:

"He's not dying. Yes, he has Parkinson's and his mobility isn't what it used to be, but he's still here."

Despite the many health problems that accompanied him in recent years, Ozzy took the stage for the last time on July 5, for a special performance with Tony Iommi, Geezer Butler and Bill Ward – the original members of “Black Sabbath” – in Birmingham, England.
In addition to his extraordinary fame in music, Osbourne became a beloved and controversial figure on screen, through the reality show “The Osbournes” on MTV (2002–2005), which opened a new era in the famous family television.

John Michael "Ozzy" Osbourne will be remembered not only for his voice and rebellious spirit on stage, but also for his resistance to illness and his intense life until the end.
