From her first day in office, Minister Evis Sala appeared with the promise that she would “complete the list of vital medicines for chronic patients.” In reality, changes to the reimbursement scheme have excluded the most necessary medicines and introduced politically connected companies, including the Novartis firm of Democratic MP Kliti Hoti, which now supplies the country’s hospitals and pharmacies with direct government approval.
On her first day as Minister of Health, Evis Sala declared that “one of her first priorities” would be to complete the list of reimbursable drugs for chronic patients. In her speech, she said that she would have “the full support of Prime Minister Edi Rama” for this process, which would improve the quality of treatment in the public system.
But just two months later, the documents show another reality: instead of vital drugs that are missing in hospitals, the minister has changed the reimbursement scheme by introducing new privileged companies, among them “Novartis”, owned by the DP MP in Shkodra, Kliti Hoti.

On the reimbursement list of the Mandatory Health Insurance Fund (FSDKSH) for 2025, the company “Novartis” had 53 types of medicines approved for sale in pharmacies and hospitals.
In October 2025, by decision of Prime Minister Rama and order of Evis Sala, the number increased to 78 medicines; an addition of 25 new medicines, all contracted by the Ministry of Health at market prices and with a guarantee of full profit.
Sources within the Health Insurance Fund confirm that the actual quantity of Novartis medicines circulating in the hospital system has increased tenfold in a few weeks, while other companies have been left out or are limited to quotas.
In practice, this means that the Ministry of Health buys the medicines from the DP MP at full market price, to distribute them to pharmacies and hospitals as reimbursable medicines, a scheme that guarantees a secure profit for the supplier, without fair competition.
The problem does not end with conflicts of interest. According to SPAK's investigative files, MP Kliti Hoti is linked to former Minister of Health Ilir Beqja, in the network of import of contraband and counterfeit medicines.
In the investigations into the pharmaceutical network that manipulated tenders for medicines and medical materials, Hoti emerges as a beneficiary through the company "Novartis" and as a secret partner of several pharmaceutical deposits, managed by his brother-in-law, Dorian Sadiku.
Police reports show that on December 10, 2021, at the Port of Durrës, 2 tons of contraband medicines destined for Hoti's company were seized, while on December 17, 2024, in Rinas, another shipment of 1 million tablets was stopped, some of which were used to prepare synthetic drugs.
However, no action has been taken against the DP MP, who enjoys complete political immunity. Local sources in Shkodra claim that Hoti’s “protection” is guaranteed by tacit agreements between Rama and the DP, which include election financing and the distribution of tenders.
The fact that Evis Sala, a minister personally appointed by Rama, has signed the increase in Novartis contracts only strengthens the suspicion that this is a political bargain with mutual financial interest.
Ironically, while companies suspected of counterfeiting and smuggling are included in the list of reimbursable drugs, vital, high-priced medicines for the chronically ill have not yet been included. Patients with diabetes, heart disease or cancer continue to pay for treatments that should have been covered by the state.
If this new “reimbursement” policy is a symbol of the reform brought by Evis Sala, then her ministry has become an instrument for enriching smugglers and keeping alive the political relations between the SP and the DP through the pharmaceutical system.
