While cancer patients wait for vital medications, dialysis is delayed, and the Oncology Department is suffocating from humidity and infections, the Ministry of Health declares hormonal treatment for transgender people at the QSUT as a priority.
In a health system where cancer patients wander through hospital doors to find chemotherapy, where drugs turn out to be expired in warehouses, and where Oncology wards are described by the High State Control as a source of infection, the Ministry of Health seems to have found a new priority: hormonal therapy for transgender people.
For the first time in Albania, the public system will offer hormonal treatment for transgender people at the QSUT, while the costs will be covered by public funds.
The decision comes at a time when the official reports of the Albanian Supreme Audit Institution itself speak of a dramatic situation in Albanian hospitals, with a lack of vital medicines, interrupted therapies, out-of-function equipment, and patients risking their lives due to chaos in the system.
The treatment will be accessible to transgender men transitioning from female to male through the hormone testosterone, as well as to transgender women transitioning from male to female through estrogen and anti-androgens.
As a rule, therapy can begin from the age of 16, while in special cases, and only with specialized assessment, it can be considered from the age of 14.
" The timing depends on the cognitive maturity of the adolescent and his ability to accept the potentially irreversible effects of the treatment. The assessment of this is complex and requires the full cooperation of the adolescent, his family and the treatment team. The possible impacts of the treatment on the health and social functioning of the child must also be taken into account ," the Ministry of Health order states.
So, while patients with tumors do not find basic medications to survive, the state is already organizing the mechanism for hormonal gender change.
The irony becomes even stronger when compared to the overview that the Supreme State Audit Office itself has released about the QSUT and the Albanian pharmaceutical system.
According to the audit by the Albanian Supreme Audit Office, out of 253 anti-tumor drugs authorized in Albania, 116 were never imported during the years 2021-2025. The report highlights the lack of vital chemotherapies, expired drugs in the system, duplicate prescriptions, lack of laboratory control, and patients who remain without treatment for weeks.
At the same time, the report reveals that in the Oncology Department, patients are treated in humid, hot, and degraded environments that are considered a potential source of infection. The basement of the building is described as completely non-functional, filled with discarded equipment, dirt, and microbiological hazards.
While dialysis or transplant patients are reported to have not received their medications on time, the Ministry of Health has decided to guarantee detailed hormonal monitoring for gender transition, with periodic tests and ongoing medical follow-up.
The KLSH report also raises the alarm about the quality of medicines entering Albania, highlighting a lack of laboratory tests and serious weaknesses in pharmaceutical control. According to the audit, the laboratory responsible for the tests has not carried out the necessary controls for a large part of the medicines introduced into the market.
But as the system faces a shortage of medicines for the most serious illnesses, health leaders have chosen to promote a new service that affects a much more limited number of patients.
In Albania, where cancer patients are forced to buy their own medicines, where chemotherapy is unavailable for months, and where official reports speak of massive mismanagement at the QSUT, the decision on hormone therapy for transgender people is being seen as another example of the healthcare system's inverted priorities. /Pamphlet
