High costs limit the laboratory concession service in hospitals, there is a risk

"Upon receiving the notification for reaching 75% of the monthly budget ceiling, the hospital takes the measures for good management defined in the concession contract".

This is one of the points of the contract between the Compulsory Insurance Fund (MFSDKSH) and the "Mother Teresa" Hospital Center valid for this year (see at the end part of the FSDKSH-QSUT contract for 2023).

This restriction has become the reason that laboratory tests are often not offered in the wards of the "Mother Teresa" Hospital Center in the last days of the month after the monthly schedule of examinations is exhausted, which is detailed based on the available funds.

Initially, the routine analyzes are interrupted, but there are many cases that in the last days of the month, even emergency analyzes were not offered, affirmed the reliable sources verified at the QSUT.

Because they bear criminal liability, the medical staff at QSUT cannot authorize laboratory tests when the monthly plan is fulfilled before the end of the month, thus leaving patients without one of the basic services.

Despite the fact that the government increased the financing of the concessionaire by 23 percent last year, bringing it to 1.6 billion ALL from 1.3 billion ALL in the initial contract, still QSUT continues to have deficiencies in this type of service.

According to the contract between FDSKSH and QSUT, it is determined that "the hospital and health personnel (referral doctors) are responsible for the realization of the Concessionary contract of the laboratories signed between the Authority (MSHMS) and the Concessionaire.

The referring specialist doctor is responsible for the referral of the examinations for the Concessionaire of the laboratories by completing the referral form based on the Clinical Practice Protocols approved by the Ministry of Health and Welfare and in implementation of the referral system. The Laboratory Analysis Referral Forms must be completed and signed by the referring physician, according to all the sections defined by the Laboratory Concession Contract and the legal acts in force".

The contract further stipulates that "the hospital has the obligation to implement the budget ceilings and payments according to the provisions in the concession contract". These obligations are leading to a lack of laboratory services in some cases where the health of patients is seriously endangered.

Concessional contracts in the health sector are proving to be problematic, as their award was made in the absence of accurate studies and projections, making them problematic in terms of financial predictability.

The contracts are already under investigation by SPAK and a series of officials who played a role in the sterilization contract have been issued restraining orders. The sterilization portion exceeded the costs of human resources and anesthesia that usually account for the largest share of operating costs anywhere in the world.

The laboratory concession of the university, regional and municipal hospitals of Saranda and Lushnja" was signed at the meeting of April 24, 2019 by the government and entered into force in December 2019. The value of the contract is 13 billion ALL and its term ends in 2028 .

For the period 2020-2022, the company received total payments from the state budget of 2.74 billion ALL, of which 1.1 billion ALL in 2021 and 1.6 billion ALL in 2022./Monitor
